Restaurant Vendor Food Safety Application Requirements

All participating restaurants must submit their Temporary Food Event (TFE) Vendor Applications and required documentation to the Public Health Officer by Monday, March 9.

Please complete the following checklist to ensure your application is complete and ready for submission.

Required Forms & Documentation

☐ Food Safety & Equipment Request Form
Complete the Food Safety & Equipment Request Form online.

☐ Signed TFE Vendor Permit
Download and complete the attached TFE Vendor Permit. The permit must be signed by the individual holding the Food Manager Certificate.

☐ ANSI-Accredited Food Manager Certificate
Provide a copy of a valid ANSI-accredited Food Manager Certificate. This must be a Food Manager certification, not a food handler certificate.

☐ Health Department Permit to Operate or Seller's Permit
Submit a copy of your Health Department Permit to Operate or Seller's Permit.

Submission Deadline

All applications and required documentation must be submitted by Monday, March 9.
